Lightly spray the inside of your slow cooker using nonstick cooking spray to make cleaning easier.

Put the chicken breasts, in a single layer, into the bottom of the slow cooker. If very thick, cut them in half horizontally to ensure even cooking.

Slice deli ham into slices and place over the chicken, cutting or tearing as necessary to cover each piece.

Lay the Swiss cheese slices on top of the ham, ensuring each piece of chicken has a proper covering.

Combine cream of chicken soup and milk in a small bowl and whisk until smooth. Evenly pour this mixture over the chicken, ham, and cheese.

Bake on LOW 4-5 hours or HIGH 2-3 hours, until the chicken is cooked through and tender (inside should reach 165°F).
